Welcome students and parents!
As we begin a new school year, please be advised that a written prescription is needed for any medication
required to be given during school hours. This includes any student carrying an Epi-pen or Inhaler for asthma.
If your son or daughter needs Tylenol/Motrin/Advil, etc. occasionally, please have a prescription or Authorization for Medication form signed by a Doctor and sent in or faxed to my office.
Our School District requires a Physical Exam for all 9th and 11th grade students. Please send in a recent Physical Exam for your son or daughter. The School Doctor will be scheduled for Physicals on all students who have not had a recent Physical Exam.
Vision and Hearing tests and screening for Scoliosis will also be done on all 9th and 11th grade students.
